There are a few possibilities. Changes in his other routines or in yours can lead to confusion on his part, temporary forgetfulness on his part or possibly a urinary tract infection. If it happens again you should reinforce the same training techniques that taught him to begin with and check with your vet about a urinalysis to rule out infection. Also if he is not yet neutered you can see some accidents as he approaches puberty.
